Registration and Trial Class Form

Policy

All students must complete the registration form and provide accurate and up-to-date personal information.

Registration is on a first-come, first-served basis, subject to class capacity.

By registering, you and your child(ren) agree to the Photo Consent and Release Policy in the following:

The parent/guardian hereby grants permission for the use of my and my child(ren)'s image, likeness, and sound of my and my child(ren)'s voice as recorded on audio or video tape, without payment or any other consideration. I understand that these images may be edited, copied, exhibited, published, or distributed. I waive the right to inspect or approve the finished product in which my and my child(ren)'s likeness appears. Additionally, I waive any right to royalties or other compensation related to the use of my and my child(ren)'s image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.

Photographic, audio, or video recordings may be used for the following purposes:

  • Conference presentations
  • Educational presentations or courses
  • Informational presentations
  • Online educational courses
  • Educational videos
  • Social media
  • And other marketing-related activities

By signing this release, I acknowledge that I grant permission for photographic or video recordings of myself and my child(ren) to be electronically displayed via the Internet or in the public educational setting.

I will be consulted regarding the use of the photographs or video recordings for any purpose other than those listed above.

This release has no time limit on its validity, and there are no geographic limitations on where these materials may be distributed.

The registration fee is $50 per student.

Registration fees are non-refundable.

Trial classes are available to new students as an opportunity to experience the program before committing to full registration.

The institute reserves the right to cancel or reschedule trial classes due to unforeseen circumstances.
